RN (Registered Nurse) Hospice Case Manager
Full Time & PRN Availability in Kansas City, MO and surrounding areas.
Overview
Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. They work with the interdisciplinary team to develop and implement a patient/family-centered plan of care, facilitating goals-of-care discussions and guiding healthcare decisions to enhance quality of life.
Key Responsibilities
Provide education to patients and family members regarding the process, plan of care, and guidance with healthcare decisions.
Consult with the attending physician and Hospice Medical Director regarding changes in a patient’s condition that require order changes or adjustments to the plan of care.
Coordinate hospice care by working with other team members and patients/family members to ensure needs are met.
Conduct initial and ongoing comprehensive assessments.
Perform evaluations for hospice care and informational visits.
Requirements
Moz. RN License
Hospice experience preferred but not required.
Valid driver’s license and auto liability insurance.
