As a RN Case Manager You will Assess/monitor physical, emotional, and psychological needs of patients
Create hospice care plans that align with the patients’ wishes and goals
Direct nursing care: administering medications, treatments, and interventions
Provide pain and symptom management
Educate and support the patient’s family and caregivers
Collaborate with interdisciplinary team
Maintain accurate and timely documentation
Participate in on-call rotation as required by local branch

Qualifications Graduate of an accredited nursing school with current licensure to practice in state of operation
One year nursing experience required, 3+ years preferred
Experience in hospice or a similar setting preferred
Knowledge of the hospice philosophy of care
Commitment to clinical and documentation excellence
Must be a licensed driver with an automobile that is insured in accordance with state/organizational requirements and is in good working order
