Overview OPWDD RN Case Managers needed to provide Case Management to the developmentally disabled within OPWDD group homes.
3 to 5 days a week with some flexibility in regard to the hours and days a week.
Responsibilities Updating and maintaining the individuals monthly MARS and health records as required by OPWDD.
Schedule and follow up on the individuals medical appointments.
Request MD orders or extensions on existing orders or extensions when necessary.
Be available to triage individuals if anything should arise while you are in the group home.
Body checks/Assessments when needed as well as following up with nursing notes.
Supervise med passes as needed for AMAP Certifications.
