Position Summary RN Nurse Case Manager at Bee Line (Support, Inc.) a progressive organization serving individuals with developmental disabilities across Colorado. This full?time role supports a caseload of individuals with intellectual and developmental disabilities, coordinating medical care, collaborating with interdisciplinary teams, and providing training and advocacy for families and staff.

Reports to: Associate Director of Nursing
Key Responsibilities
Delegate and consult with unlicensed providers, families, and Support, Inc. staff.
Provide medical consultation to interdisciplinary team members.
Teach and train providers, families, and staff in medication administration, documentation, assessment of illness signs, basic health skills, and use of specialized medical equipment.
Apply nursing practices, state and federal regulations, and guidelines to ensure compliance with home and community?based services.
Complete and document evaluations and assessments to identify training and support needs.
Develop care plans, review medical documentation, and track health metrics.
Maintain effective working relationships across teams throughout the case management process.
Advocate for individuals and caretakers in clinical decision?making.

Qualifications RN with current Colorado license.
Minimum two years of professional healthcare experience.
Preferred: Experience as a registered nurse working with individuals with intellectual and developmental disabilities.
Knowledge of physical conditions and treatment protocols for the IDD population.
Experience in nurse case management.

Work Environment & Schedule Primarily a work?from?home position with occasional travel to offices (Aurora, Colorado Springs, Pueblo) and client homes across the front range. Requires a personal vehicle for transportation. Schedule: Monday?Thursday, 10 hours/day; flexible scheduling that may vary based on need. No weekend or on?call phone required.
Language, Reasoning & Physical Demands Proficient in reading, interpreting regulations and technical procedures; able to write reports and present information to managers and families. Must be able to prioritize tasks and manage time efficiently. Job duties involve frequent driving, lifting up to 50 pounds, and frequent use of hands and vision.
