Position Overview
Coordinate and oversee the medical care and treatment of individuals with intellectual and developmental disabilities, collaborating with physicians, interdisciplinary teams, and families to promote positive outcomes. Apply clinical knowledge to assess individuals, provide consultation, and train staff.
Responsibilities
Provide delegation and consultation to unlicensed providers, families, and Support Inc staff.
Offer medical consultation to interdisciplinary team members.
Teach and train providers, families, and staff on:
Medication administration (QMAP) and MRB training.
Individual specific medical documentation.
Evaluating signs of illness or dysfunction that warrant medical or nursing intervention.
Basic skills required to meet the health needs of the individual.
Utilizing specialized medical equipment.
Apply knowledge of nursing practices, state and federal regulations, guidelines, and procedures to ensure compliance with home and community-based services.
Complete and document evaluations and assessments to identify training and support needs for continuity of care.
Develop care plans, review medical documentation, and track health metrics.
Develop effective working relations and cooperate across teams throughout the case management process.
Advocate for individuals and caretakers by providing informed decision-making regarding clinical status and treatment options.
Experience as a Registered Nurse working with individuals with intellectual and developmental disabilities.
Knowledge and experience of physical conditions and treatment protocols for the IDD population.
Nurse case management experience.
Work Environment
This role is primarily a work‑from‑home position, requiring occasional travel to Aurora, Colorado Springs, Pueblo, and clients’ homes across the front range.
Schedule
Monday‑Thursday, 10 hours per day. Flexible schedule may vary based on need. No weekend or on‑call phone required.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from managers, families, and the public.
Reasoning Ability
Ability to solve practical and complex problems and critically reason through a variety of variables in situations where limited standardization exists. Interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Prioritize work needs to manage time.
Physical Demands
Regularly required to reach with hands and arms, talk or hear. Frequently required to drive. Occasionally lift and/or move up to 50 pounds. Vision abilities: close vision, distance vision, color vision, depth perception, adjust focus.
