RN - Hospice Visit & Triage Nurse Position Title: RN - Hospice Visit & Triage Nurse
Location: Upland Hills Health - Hospice Department - Dodgeville
Role & Department: Registered Nurse in Home Health Services Department, Hospice area.
Hours & Shift: Part?time (.8 FTE) Day shift, weekend/holiday rotation, on?call responsibilities.
Role Responsibilities
The RN will provide comprehensive, compassionate nursing care for hospice patients in homes, assisted living, or long?term care settings.
Assess, plan, coordinate, and deliver individualized care focused on comfort and quality of life.
Communicate with physicians and other medical team members as appropriate.
Respond to on?call needs of hospice patients.
Collaborate closely with interdisciplinary team to ensure physical, emotional, and spiritual needs are met.
Provide direction and oversight for LPNs and CNAs caring for patients.
Assess needs of each patient assigned.
Administer medications and treatments as ordered, ensuring comfort and symptom management.
Provide education and emotional support to patients and caregivers regarding disease process, medication management and end?of?life care.
Plan and prepare nursing care plan and follow through.
Provide skilled nursing care to ensure comfort, respect and love.
Document all assessments, interventions, and communications accurately and promptly in EMR.
Advocate for patients and families, promoting dignity, comfort, and respect.
Triage support: professional nursing support and coordination for patients during daytime hours.
Ensure timely support, comfort, and guidance to patients and families.
Serve as communication hub among patients, caregivers, field nurses, and providers.
Conduct thorough phone assessments to evaluate symptoms, medications, and patient needs, coordinating appropriate follow?up.
Manage medication refills, durable medical equipment orders, and supplies in coordination with pharmacy and vendors.

Qualifications Graduate of an accredited nursing school.
Licensed as a Registered Nurse in Wisconsin.
Minimum of one year of experience in home health, hospice, or palliative care (preferred).
Strong verbal and written communication skills.
Familiarity with hospice regulations and EMR documentation.
Driver's ability to travel long distances and enter varied home environments.
Ability to plan, direct, and evaluate nursing care.
Interest in professional growth, continuing education, and participation in professional organizations (preferred).
