Overview
Compassionate Care Hospice, an Amedisys Company, is seeking a dedicated RN Case Manager Float Nurse to join our exceptional team in Lakeland, FL. This full‑time position (M‑Friday, 9 am‑6 pm) requires travel within Polk County territory.

Location: Lakeland, FL (travel within Polk County)

Schedule: Monday–Friday, 9 am–6 pm

Salary
$80,000–$85,000 annually, with additional bonus opportunities.

Final compensation may vary based on location, job‑related knowledge, skills, and experience.

Orientation, Onboarding & Training
Orientation for an RN averages 7‑9 weeks, individualized according to your needs and experience.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.

Make referrals to other hospice care disciplines as needed or document rationale for not doing so.

Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness.

Record pain/symptom management changes and outcomes.

Teach patients, families, and caregivers appropriate patient care techniques.

Regularly communicate with the clinical manager and care team.

Perform on‑call responsibilities and provide on‑call services to patients/families as assigned.

Other duties as assigned.

Qualifications

Current RN license specific to the state(s) you are assigned to work.

At least 1 year(s) of clinical RN experience in an acute healthcare setting – required.

Current CPR certification.
