RN Hospice Case Manager – Amedisys Home Health
Location: Lakeland, FL (daily travel within Winter Haven Facilities/Field Territory). Schedule: Monday–Friday, 8 am–5 pm.

This full‑time case manager role coordinates hospice nursing and palliative care for terminally ill patients at home, ensuring high‑quality end‑of‑life care and extensive collaboration with physicians, families, and interdisciplinary teams.
Pay
$80,000 – $85,000 plus bonus opportunities. Compensation may vary by location, experience and related qualifications.

Life as a Hospice RN Case Manager
You will coordinate, direct, and support hospice nursing and palliative care, work closely with physicians, and lead interdisciplinary team meetings to achieve excellent pain and symptom management.
Nuts & Bolts of the Job
Manage 12–14 patients, attend interdisciplinary team meetings.
Assign aide visits and coordinate care with social workers, chaplains, volunteers, and bereavement teams.
Develop relationships with facility staff and community leaders.
Responsibilities
Perform patient assessments and collaborate with the care team to develop and implement care plans.
Make referrals to other hospice care disciplines as needed, or document rationale for not doing so.
Prepare and maintain individualized patient care progress records accurately and timely.
Record pain/symptom management changes/outcomes as appropriate.
Teach patients, families, and caregivers appropriate care techniques.
Communicate regularly with the clinical manager and care team.
Perform on‑call responsibilities and on‑call services to patients/families as assigned.
Other duties as assigned.
Qualifications
One year of nursing experience.
Current RN license, specific to the state(s) you are assigned to work.
Current CPR certification.
Valid driver’s license, reliable transportation, and liability insurance.
