This position is based in Lakeland, FL and requires daily travel within the Winter Haven Facilities/Field Territory. It is full‑time, M‑Friday 8am – 5pm.

Life as an Hospice RN Case Manager
As a Hospice Case Manager for Amedisys, you will have a special opportunity to provide, coordinate and direct the provision of Hospice nursing and palliative care to terminally ill patients in their home and wherever they find comfort. Our Hospice nurses will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high‑quality end‑of‑life care for the patient. We serve an adult population providing support to patients and families.
Nuts & Bolts of the Job
Hospice RN Case Managers generally work a Monday to Friday schedule. Flexible and creative scheduling options may be available based on the needs of the care center, including potential on‑call and holiday coverage. Case manage 12‑14 patients and attend interdisciplinary team meetings. Provide case management by:
Assigning aide visits.
Coordinating care with social workers, chaplains, volunteers and bereavement team.
Developing and nurturing relationships with facility staff and leaders throughout the community.

Orientation, Onboarding & Training
You will then continue your onboarding working directly with nurses in our program. On average our orientation for an RN is 7‑9 weeks, however it is individualized according to your needs and experience.

Caring for patients and families at end of life can create challenging situations.

On‑call rotation can get busy if the primary On‑Call RN is on PTO or the seat is temporarily vacant.
Responsibilities
Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
Records pain/symptom management changes/outcomes as appropriate.
Teaches patients, families, and caregivers appropriate patient care techniques.
Regularly communicates with the clinical manager and care team.
Performs on‑call responsibilities and on‑call services to patients/families as assigned.
Other duties as assigned.
Qualifications
One year of nursing experience.
Current RN license, specific to the state(s) you are assigned to work.
Current CPR certification.
Valid driver’s license, reliable transportation and liability insurance.
