We have an immediate opening for a Home Health RN Case Manager at Oak Brook, IL
Position Details:

• Pay: $67/hr
• Shift: Mon-Fri, 8 AM - 5 PM (Every 3rd weekend)
• Fully Field-Based - No office reporting
• Service Areas: Aurora, Montgomery, Oswego, Yorkville, Plainfield & nearby

Key Responsibilities:
Manage 20-30 patients/week (6 visits/day)
Provide wound care, IV therapy, ostomy care, & blood draws
Care for patients with multiple comorbidities
EPIC documentation (laptop provided)

Requirements:
1+ year Home Health RN experience
Active IL RN license & BLS (AHA)
Wound care, IV, & blood draw experience
Reliable transportation & valid driver's license
