RN Home Case Manager - Summit and Cuyahoga Counties
Join the Cleveland Clinic team as an RN Home Case Manager serving Summit and Cuyahoga Counties. This role involves providing patient‑first home health nursing care, including assessments, complex nursing treatments, education, and coordinating patient care to help avoid readmissions to acute care settings. The Home Health RNs work remotely after patient visits to complete documentation, make calls, attend meetings, coordinate care, and arrange visit schedules. The position includes an in-depth orientation tailored to experience level and is eligible for student loan repayment.
Responsibilities
Provide direct nursing care.
Perform accurate admission assessments and reassessments within specified time frames.
Assess patients for safety risks, abuse, and neglect.
Serve as primary coordinator of all disciplines for well‑coordinated patient care.
Minimum qualifications
Graduate from an accredited school of Professional Nursing
Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS) Certification through the American Heart Association (AHA)
Valid Ohio Driver's license upon hire and per policy 8-175 thereafter
Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter
One year of current experience as an RN
One year of experience in an acute Medical/Surgical or Home Care setting
Preferred qualifications
Bachelor’s of Science in Nursing (BSN)
Experience working in home care, medical/surgical, ICU and/or ED areas
Work schedule and location
A caregiver in this position typically works days from 8:00am – 5:00pm with requirements for weekends, holidays, and some on‑call coverage.

Physical requirements
Requires full range of motion, manual and finger dexterity, and eye‑hand coordination
Standing and walking for extended periods
Corrected hearing and vision to normal range
Exposure to potential communicable diseases or bodily fluids
Medium work; ability to move objects and assist patients as needed
Ambulatory and able to drive to patient homes, climb stairs, and assist patient movement
Personal protective equipment
Follows standard precautions and uses PPE as required
