Overview Position: Registered Nurse – Field Case Manager. Location: service area within the Tri-County area (Oakland, Macomb & Wayne counties), with the corporate office in Southfield, MI. The RN administers skilled nursing care to patients in their place of residence in accordance with physician orders and the plan of care under the direction of the Clinical Manager/Nursing Supervisor.
Responsibilities Provides services in accordance with the plan of care.
Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
Initiates the plan of care and necessary revisions.
Provides services requiring substantial specialized nursing skills.
Initiates appropriate preventive and rehabilitative nursing procedures.
Prepares clinical and progress notes for each patient visit and summaries of care conferences in a timely manner as per Agency policy.
Coordinates services and informs personnel of changes in patient condition.
Counsels the patient and family/significant others in meeting nursing and related needs.
Participates in in-service programs, peer reviews, and performance improvement activities as assigned.
Adheres to established Agency policies and procedures; processes orders and notifies physicians of patient needs and condition changes; completes certification/recertification orders and discharge summaries.
Determines the amount and type of nursing needed for each patient; refers to Physical Therapy, Speech-Language Pathology, Occupational Therapy, and Medical Social Work as needed.
Supervises and teaches other nursing personnel; conducts patient care conferences for patients assigned to the nurse’s care.
Participates in utilization review of medical records and completes OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant changes in condition within defined time frames.
Appropriately utilizes ICD-10 codes; takes on-call duty nights, weekends and holidays, as assigned.
Qualifications Graduate of an approved school of professional nursing and currently licensed in the state of Michigan
One year of nursing experience
Kinnser – EMR experience preferred
Home Healthcare experience preferred
Experience in wound VAC, PICC line and mediport care preferred
Pediatric experience desired
Working Environment Works indoors in the Agency office and patient homes and travels to/from patient homes.
