Provide care in Oakland and Wayne County, MI.

Qualifications Graduate of an approved school of professional nursing and currently licensed in the state of Michigan.
Minimum one year of nursing experience.
Experience with Kinnser – EMR preferred.
Home healthcare experience preferred.
Experience with wound vac, PICC line, and mediport care preferred.
Pediatric experience desired.
Responsibilities Provide services in accordance with the plan of care.
Make the initial evaluation visit and regularly re‑evaluate the patient's nursing needs.
Initiate the plan of care and necessary revisions.
Provide services requiring specialized nursing skills.
Initiate appropriate preventive and rehabilitative nursing procedures.
Prepare clinical and progress notes for each patient visit and summaries of care conferences in a timely manner as per agency policy.
Coordinate services and inform personnel of changes in the condition and needs of the patient.
Offer counsel to patient and family/significant others in meeting nursing and related needs.
Participate in and present in‑service programs.
Understand and adhere to established agency policies and procedures.
Process orders and notify the physician of patient needs and changes in condition; complete certification/recertification orders and discharge summaries.
Determine the amount and type of nursing needed by each individual patient.
Refer to PT, ST, OT, and medical social worker for patients requiring specialized skills.
Supervise and teach other nursing personnel.
Conduct patient care conferences for patients assigned to your care.
Participate in peer review and performance improvement as assigned.
Participate in utilization review of medical records as assigned.
Provide total patient care as needed.
Takes on‑call duty nights, weekends, and holidays as assigned.
Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with agency‑defined time frames.
Appropriately utilize ICD‑10 codes.
Working Environment Works indoors in the agency office and in patient homes and travels to/from patient homes.
