Clinical Care Manager – OBAT RN, Framingham, MA
Salary Range: $32.00–35.00/Hour (part‑time 20 hours/week, can flex into a full‑time opportunity). Requires a Massachusetts Nursing License.

We are currently hiring a Clinical Care Manager (CCM) – OBAT RN based in Framingham. As part of a team‑based approach to care, the CCM provides, coordinates, and organizes evidence‑based care management to MAT program patients.

Essential Functions:

Assist Primary Care Providers (PCPs) in coordinating and managing the care of MAT program patients, ensuring optimal care—including acute illness and chronic disease management, lifestyle and behavior modification education, and preventive care across multiple health settings.

Manage care plans for patients admitted to and discharged from the hospital, seen in the emergency room, or transitioning between health care facilities, with the goal of preventing disease exacerbation, improving outcomes, increasing patient engagement in self‑care, reducing risk status, and minimizing hospital and ER utilization.

Identify, manage, and coordinate patient care and provide ongoing communication to the care team regarding patient needs, plans of care, and changes in status.

Provide direct patient care within the nursing scope of practice, including patient assessment, administration of vaccinations and medications, medication reconciliation, point‑of‑care testing, treatments, and review of lab and diagnostic test results.

Provide timely and informed patient triage.

Involve the patient and their support systems in communication, care coordination, care planning, and education.

Note: This position may require evening, weekend, and/or holiday shifts on a rotating basis.

Required Qualifications:

Registered Nurse License in MA

Bachelor’s degree in Nursing from an accredited college or university

1–2 years of clinical experience

Current CPR certification
