Community Healthcare of Texas is currently recruiting for an RN Case Manager.
$3,000 Sign-On Bonus
POSITION SUMMARY
Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.
ESSENTIAL FUNCTIONS
Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.
Case manage total care of the patient by coordinating with team members/physicians, supervising CNA activities, and participating in staff and Interdisciplinary Team meetings.
Complete daily paperwork and charting, thoroughly and accurately according to Policy.
Participate in on-call rotation.
May assist with admissions
QUALIFICATIONS
Registered Nurse in the state of Texas
Hold a valid driver’s license and have a dependable means of transportation
1 year of nursing experience
2+ years of experience in hospice case management strongly preferred
Proficient skill using a computer is required; Microsoft office suite experience is preferred.
