Suncoast Hospice, a member of Empath Health, is seeking a (PRN) Registered Nurse Case Manager to join the Empath team.
Coverage area is Hillsborough County. Bilingual - fluent in English and Spanish preferred.
The (PRN) RN Case Manager is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the terminally ill patient/family unit. They utilize assessment, intervention and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.

Position Requirements Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; Minimum of one year related experience and/or training; or equivalent combination of education and experience.
Licensure: Valid Florida license as a Registered Nurse
Valid Florida State Driver’s License

Prefer one (1) year experience in an acute or post acute care setting
