Position Summary
The RN Case Manager (Float) is responsible for planning and managing the nursing care of an assigned caseload of patients in home or long‑term care settings. In collaboration with the interdisciplinary team, the RN Case Manager ensures that patient‑ and family‑directed goals are achieved using a holistic, patient‑centered approach. The role includes providing education, facilitating communication, and coordinating care across various sites. Flexibility in work location across care settings is required.
Essential Functions
Assess and identify patient and family goals and needs.
Collaborate with interdisciplinary team members to develop and implement individualized care plans.
Participate in team meetings and ethical discussions.
Educate patients and caregivers about disease progression, medications, nutrition, and hospice philosophy.
Coordinate timely and effective responses to changing or urgent patient needs.
Maintain accurate, timely, and compliant documentation.
Support and educate team members, patients, families, and the broader community.
Participate in internal quality improvement initiatives.
Facilitate interdisciplinary group (IDG) meetings in the absence of the manager.
Perform other duties as assigned.
Qualifications
Knowledge and Critical Skills
Effective verbal, written, and non‑verbal communication skills.
Understanding of hospice and palliative nursing principles.
Cultural competence and ethical decision‑making in care delivery.
Ability to set professional boundaries and resolve team conflicts.
Strong critical thinking and clinical judgment abilities.
Proficient in time management, organization, and accountability.
Knowledge of infection control and standard precautions.
Experience
Minimum of one year of Registered Nurse experience.
Preferred: One year of experience in a hospital medical‑surgical or oncology setting.
Acceptable alternative: Five years of Licensed Practical Nurse experience within the past five years, including one year of hospital medical‑surgical or oncology experience.
License / Certification
Current Registered Nurse license in the State of Florida.
Valid CPR certification.
Current driver’s license with an acceptable driving record.
Education
Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program.
Ability to work extended and irregular hours as needed.
Ongoing participation in professional development and continuing education.
