Position Summary The RN Case Manager is responsible for coordinating the care of patients throughout their hospital stay, ensuring they receive optimal, timely, and efficient care. They work collaboratively with multidisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure that patients’ medical, social, and emotional needs are met. The RN Case Manager also focuses on optimizing resource utilization, improving patient outcomes, and preventing unnecessary hospital readmissions.
Education Graduate from an accredited school of nursing, bachelor’s degree preferred.
Experience Three years' experience in case management or utilization management preferred. Registered Nurse with at least one year experience in medical/surgical with excellent communication skills and ability to work collaboratively with all members of the health care team required. Requires physical stamina and emotional stability.
Licenses/Certificates Current State of Illinois RN License.
Compensation Hourly Range: $31.25-$46.88
Incentives Eligible for evening/night, weekend shift incentive based on company policies and applicable job codes. Additional details will be provided during the interview process.
