Job Overview
Good Shepherd Hospice is seeking a compassionate candidate to serve as an RN Case Manager (PRN) supporting patients and families in Dallas and surrounding communities. The RN provides nursing assessment, planning, and care to maximize comfort and health of patients and families, responding to all phone calls promptly, making patient visits as needed, and completing after‑hours admissions and deaths.
Responsibilities
Receives patient assignments from the Patient Care Manager or Case Manager; initiates communication with attending physicians, hospice staff members and other agencies to coordinate optimal care.
Cultivates communication and bereavement skills; maintains regular communication with physicians and families.
Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporates that data into the Plan of Care.
Maintains up‑to‑date patient records, reflecting changes in problems, plans, actions and goals; completes required paperwork per Medicare and Hospice policies.
Instructs patients and caregivers on medication administration, side effects, personal care, hygiene, lifting, ambulation, safety, and pre‑death preparation.
Informs the Patient Care Manager of any unusual or potentially problematic patient/family issues.
Attends hospice team meetings and patient conferences as needed.
Provides support at time of death and shares in 24‑hour, seven‑day coverage of patients and families.
Evaluates hospice aides every two weeks; evaluates LPNs at least once per month.
Through individualized studies, learns and develops an understanding of hospice philosophy; becomes proficient in pain management.
Performs all other tasks assigned.
Qualifications
Registered Nurse (RN) license in good standing.
Previous experience in hospice, home health, or geriatrics preferred.
Strong assessment, communication, and patient‑care skills.
Ability to work independently and collaborate with interdisciplinary teams.
