Overview
We are looking for a high‑energy, compassionate, detail‑oriented Registered Nurse (RN) to be a Hospice Case Manager for our patients. As a Hospice RN Case Manager, you will plan, organize, and direct hospice care utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will effectively interact with patients, caregivers, families, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency.

Schedule
After Hours / On Call. Days and shifts will vary based on need.

Responsibilities

Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.

Partner closely with the Clinical Director to assess eligibility and suitability of clients for our specialized hospice services.

Provide comfort and support to patients, ensuring their comfort and quality of life remain paramount.

Deliver top‑notch nursing care rooted in proven principles and techniques.

Educate and advocate for patients and families, encouraging active participation in personalized care plans.

Share call duties, ensuring presence when it matters most.

Qualifications

Current, unencumbered RN license.

Minimum one (1) year experience as an RN in a medical-surgical/acute care setting; hospice experience is a plus.

Computer proficiency in typing, EMR, and related programs.

Current CPR certification if required by state.

$34.00 - $42.00 per hour (Average Pay Range)
The pay range listed is a general guideline. Final compensation will be determined based on relevant experience, qualifications, and specific responsibilities.
