Job Summary and Qualifications
The RN CM Care Coordinator facilitates interdisciplinary care, evaluates clinical appropriateness, manages resources, and supports quality outcomes and patient throughput. Responsibilities include comprehensive assessments, care planning, interdisciplinary team leadership, evaluating admissions, managing utilization, and facilitating discharge planning.
Qualifications include:
Associates Degree in Nursing or Diploma required; Bachelors preferred.
Current FL RN license or appropriate compact license.
Certification in Case Management or Nursing preferred.
Minimum three years of nursing experience in acute care; case management experience preferred.
