Job Summary

Position: RN Case Manager

Location: Vancouver, WA / Oregon Territory

Schedule: Monday – Friday (Rotating on-call; occasional evening/weekend visits)

Compensation: $78,000–$80,000 per year + quarterly incentives, mileage reimbursement, company phone

Sign-On Bonus: $5,000

Aveanna Healthcare is seeking an experienced and compassionate RN Case Manager to join our home health team in the Vancouver/Oregon area. Responsibilities

  • Oversee and promote clinical excellence among field nursing staff
  • Review and update physician orders and patient care plans
  • Conduct start-of-care assessments, re-evaluations, and discharge visits
  • Train, mentor, and support nursing team members
  • Coordinate patient care and communicate effectively with interdisciplinary teams
  • Ensure compliance with federal, state, and company policies and documentation standards
  • Participate in quality assurance and performance improvement initiatives
  • Manage internal case assignments and oversee patient outcomes

Qualifications

Required:

  • Active RN license in the state of practice (WA and/or OR)
  • Minimum 1 year of hands-on clinical nursing experience
  • Reliable transportation and valid driver’s license (MVR check required)
  • Current CPR certification (AHA or ARC)
  • Ability to travel within assigned patient territory

Preferred:

  • Experience in home health or pediatric/adult skilled nursing supervision
  • Strong clinical documentation and care coordination skills
