Job Description
Position Summary
Case managers serve as the care coordinators for patients, care givers, and families. Their functions include concurrent and retrospective review to assure appropriate utilization, assessment, prevention of denials and coordination of discharge options. They are required to follow specific criteria sets to assure regulatory compliance. They coordinate safe discharge plans between the health care delivery team, patients, families, insurers and continuing care agencies.
Education / Experience
Graduate of an accredited school of nursing
BSN preferred
Certification / License
Current Massachusetts RN licensure
Certification in Case Management preferred
Pay Range
$35.85 - $77.82
This position is covered by a collective bargaining agreement. The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. The pay range for this position is established and governed by the terms of the applicable collective bargaining agreement. Employees hired into this position will receive compensation consistent with the terms of the applicable collective bargaining agreement.
