Job Title: RN Care Manager The RN Care Manager (RN CM) utilizes advanced nursing skills, resource management, and fiscal responsibility to coordinate clinical care for a designated patient population across the continuum of care. Responsibilities include, but are not limited to, clinical effectiveness, discharge planning, care coordination, and clinical resource management. The RN CM collaborates with Medical, Nursing, and Ancillary staff to facilitate quality, cost-effective patient outcomes and ensure care is provided in the most appropriate setting.
Specifications: Must hold and maintain a Rhode Island Registered Nurse license.
Minimum of 3 to 5 years of related clinical and/or Case Management experience.
Certification in Case Management (CCM or ACM) within 24 months of employment (prorated for part-time staff).
Effective communication skills with residents and physicians regarding documentation and interdisciplinary care team collaboration.
Ability to interpret medical data and utilize level of care management, quality management, and discharge planning knowledge.
Knowledge of third-party payer or case management preferred.
Proficiency in multiple computer systems (e.g., EMR, Microsoft Office).
Participation in weekend and holiday coverage rotations with flexibility to cover as needed.
