As a Case Manager, you'll play a vital role in coordinating and overseeing the care of pediatric patients while serving as a key advocate for families. Your responsibilities will include:
Patient Assessments & Care Planning Conduct comprehensive assessments, develop individualized care plans, and adjust treatments as needed.
Care Coordination & Advocacy Collaborate with physicians, therapists, and caregivers to ensure seamless, high-quality patient care.
Family Education & Support Teach families about treatment plans, disease management, and best practices for home care.
Clinical Nursing Care Provide direct care, including medication administration, wound care, tracheostomy/ventilator management, and emergency interventions.
Monitoring & Documentation Track patient progress, recognize changes in condition, and ensure accurate documentation within the client's record.
Leadership & Team Collaboration Work effectively within a multidisciplinary team, demonstrating flexibility and problem-solving skills in various healthcare situations.
Compliance & Quality Assurance Uphold Elara Caring's mission, philosophy, and policies to maintain the highest standards of care.
Physical Requirements Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift 50-100 lbs.
Licensure Active and unrestricted RN license in the state of practice.
Transportation Reliable vehicle, valid driver's license, and current auto insurance (per state laws).
Skills & Attributes Strong clinical judgment, problem-solving abilities, and the ability to communicate effectively with families and healthcare teams.
