Job Summary and Qualifications We are seeking a Registered Nurse (RN) Case Manager who will provide clinical expertise to ensure all patients receive high-quality, efficient care. You will use your knowledge, skills and abilities to make a positive impact on patient care.
What you will do in this role:
Coordinate the plan of care for the patient stay, manage length of stay, ensure appropriate resource management, and develop a safe discharge plan.
Facilitate progression and transition of care and identify patients at risk for adverse outcomes during transitions between care settings.
Perform a comprehensive psychosocial, medical and discharge assessment of patients and families, and reassess clinical condition as indicated.
Ensure post-acute medical needs and level of care are appropriate in partnership with Social Services.
Evaluate progression of care using evidence-based tools (InterQual) and escalate when needed.
Make referrals to third-party payer, disease and case management programs for recurring patients and those with chronic disease states.
Maintain effective and professional communication between physicians, patients/families, hospital staff and outside agencies; advocate for patientsul rights, needs and values.
What qualifications you will need:
Graduate of an accredited School of Nursing: Associateegree in Nursing or higher required; BSN is highly preferred.
Licensed as a Registered Nurse in the practicing state and/or active Compact/Multi-State RN license.
1–3 years of clinical, hospital nursing experience required.
2 years of experience in case management or critical care preferred.
Certification in case management or utilization review preferred.
InterQual experience preferred.
