Overview RN Case Manager - Part time - Adena Greenfield at Adena Health System
The Registered Nurse (RN) Case Manager works collaboratively with the multidisciplinary care team to coordinate and advocate for patients across the continuum of care. This role supports patients and families through assessment, planning, and facilitation of care to ensure safe, effective, and timely transitions. The RN Case Manager focuses on inpatient and post-acute care coordination, while the ED Case Manager RN supports patient transitions from the Emergency Department to the community or appropriate care settings—optimizing resource utilization and improving patient outcomes.
Responsibilities
Coordinate and advocate for patients across the continuum of care in collaboration with the multidisciplinary care team.
Perform assessment, planning, and facilitation of care to ensure safe, effective, and timely transitions.
Focus on inpatient and post-acute care coordination; understand the role of the ED Case Manager RN in transitions from the Emergency Department.
Minimum Qualifications
Education: Bachelor’s Degree in Nursing (BSN)
License/Certification: Current Ohio RN License
Experience: Five (5) years recent clinical experience or three (3) years inpatient hospital experience
Preferred: Certified Case Manager (CCM®); prior case management experience
