We are adding a RN Case Manager to our clinical team in Vancouver, WA/Oregon . This role offers the opportunity to develop and provide professional guidance to our field staff, monitor clinical excellence, and enjoy autonomy and flexibility in scheduling. Schedule: Monday – Friday
Additional Details: Rotating on-call (may include occasional evening/weekend visits)
Work Location: Combination of Aveanna Vancouver office, Vancouver, and Oregon patient homes
Compensation: $78-79K/year plus potential monthly incentives, cellphone, and mileage reimbursement

Responsibilities: Promote and enhance the clinical expertise of staff to ensure safe and quality nursing care
Review physicians’ orders and update care plans and documentation as needed
Assist with interviewing, training, and validating new clinical staff
Participate in patient discharge planning through “Start of Care Assessments” and “Re-evaluations”
Collaborate with operations and business development teams
Internal case management and development of nursing talent
Ensure adherence to all relevant policies and procedures
Conduct quality assurance of clinical documentation
Coordinate care with ancillary healthcare providers
Qualifications: Valid RN license in the practicing state
At least 1 year of hands-on nursing experience
Ability to travel to patients’ homes within the designated territory
Reliable transportation, valid driver’s license, and pass MVR check
Current CPR certification from AHA or ARC
Willingness to supervise nurses providing in-home skilled nursing care to diverse patient groups
