Responsibilities The Case Manager, as a member of the health care team, collaborates to enhance the delivery of patient care services along the continuum of care. The case manager evaluates, predicts, and facilitates the trajectory of patient care.
Employs a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation to meet the individual’s health care needs.
Works toward continuously improving the patient’s experience, helping to ensure institutional standards of high-quality patient care, reducing cost, and ensuring reimbursement.
Participates in orientation and onboarding programs for Case Management; supports the institutional recognition programs and collaborates with leaders and subject matter experts within the department.
Contributes as part of a team to support the Case Management department and Mass General Brigham mission.
Qualifications RN Case Manager experience required
Licensed RN in the state of Massachusetts
Minimum of 2 years Med/Surg experience required
Case Manager certificate preferred
ADN required, but BSN strongly preferred
Acute hospital experience preferred
