SIGN ON BONUS OFFERED!!!! Kenosha Visiting Nurse Association is growing. We are a home health agency providing services in patient homes in Southeast Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, and any other place they may call home. Kenosha Visiting Nurse Association strives to provide high quality care through our multifaceted programs. We are seeking a Registered Nurse Case Manager.

Full-Time, 32 - 40 hours per week, M - F.

Responsibilities

Completes an initial assessment of patient and family to determine home care needs.

Develops a written Plan of Care for each patient, assessing patient\'s condition, re-evaluating and updating Plan of Care as necessary and educating patient and caregivers appropriately.

Provides skilled nursing care.

Counsels patient and family with regard to nursing and related needs.

Utilizes EHR to document patient care.

Participates in bi-weekly case conferences to discuss multidisciplinary team responsibilities and provide updates on patient status.

Identifies discharge planning needs as part of Plan of Care development and implements prior to patient discharge.

Qualifications

1 year of clinical experience in home health setting (minimum).

Graduate of an accredited RN licensing program and current RN licensure.

Ability to work independently and as part of a team.

Good organization and effective time management skills.

Compliance with documentation standards and regulations.

Strong patient assessment skills.

Excellent written and verbal communication skills.

Current CPR Certification.

Current driver\'s license, valid vehicle insurance, and access to a dependable vehicle.

Position Details

Location: Kenosha and Racine Counties.

Schedule: Full-Time, 32-40 hours per week. Rotating on-call required.

Compensation: $68,000 - $76,000.00.

Sign-On Bonus: $5,000.00.
