Overview
Altus Hospice Care is hiring a full‑time RN Case Manager Hospice to provide compassionate, detail‑oriented care in the Sugarland area. The RN Case Manager will plan, organize, and direct hospice care using the nursing process of assessment, planning, interventions, implementation, and evaluation. The role requires effective interaction with patients, caregivers, families, and interdisciplinary team members while maintaining professional nursing standards and clinical competency.

Responsibilities

Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.

Collaborate with the Clinical Director to assess eligibility and suitability of clients for hospice services.

Coordinate care as an RN Case Manager, ensuring comfort and quality of life for patients.

Deliver top‑notch nursing care rooted in proven principles and techniques.

Educate and advocate for patients and families, encouraging active participation in personalized care plans.

Participate in call duties, demonstrating commitment to being present when it matters most.

Qualifications

Current, unencumbered RN license.

Minimum one (1) year RN experience in a medical‑surgical/acute care setting; hospice experience is a plus.

Computer proficiency, including EMR background.

Current CPR certification if required by state.

Compensation
$68,000 – $76,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
