Qualifications Enthusiasm for providing the best care to patients and families
RN license in good standing in CA
Preferred: 1 year of hospice or home health experience
Valid driver’s license
Current CPR certification
Excellent observation, verbal, and written communication skills
Daily Responsibilities Create and initiate care plans based on patient assessments
Spend quality time with patients and families to support their needs
Conduct initial and ongoing assessments of patient and family needs
Record vital signs and health information
Administer medications and treatments as prescribed
Provide health care instructions to patients
Document symptom management and outcomes
Maintain accurate clinical notes
Perform additional duties as assigned
