RN Case Manager – Hospice – Days

Responsibilities
Manages and coordinates all aspects of service delivery as defined by physician orders.
Participates in interdisciplinary group meetings and the re-certification process. The Case Manager is directly responsible for all aspects of the patient’s care. The plan of care is discussed with all disciplines and reevaluated as needed. This includes needs such as comfort measures, pain management, psychosocial needs, spiritual needs, volunteer needs, and/or bereavement issues.
Ensures implementation of activities for improved quality of patient care.
Supervises Home Health Aide activities and plan of care.
Supervises and educates nursing/medical students during their rotation.
Specializes in pain and symptom management issues in patient care.
The position may have additional or varied physical demand and/or respiratory fit test requirements. Potential exposure to infections and communicable diseases, blood and body fluids, electrical equipment, chemicals. Must follow standard precautions.
Qualifications
Experience:
1 year of clinical nursing experience.
Education:
Associate’s or Bachelor’s degree.
Graduation from an approved school of nursing.
License / Certifications:
Registered Nurse (RN) current licensure as a Registered Nurse in the State of Oklahoma or current multistate license from a Nurse Licensure Compact member state.
Basic Life Support (BLS) issued by American Red Cross or American Heart Association within 30 days of hire.
Must be able to communicate effectively in English (verbal and written). This job requires the incumbent to operate a INTEGRIS-owned vehicle or a personal vehicle (non-INTEGRIS-owned) and therefore must have a current Oklahoma State Driver’s License along with a driving record acceptable to our insurance carrier.
