Pay Rate: $1,800.00 - $2,700.00

Position Summary

Admits patients to service, assesses the patients and familys physical, psychosocial, environmental, safety, and developmental needs. Develops an individualized plan of care based on assessment and documents observations and interventions related to the patients environment. Coordinates care with a multidisciplinary team, participates in the hospice programs quality assessment performance improvement program, and conducts follow?up visits as needed. Floats to various cities within the territory.

Qualifications

• CA RN license


• BLS certification


• Minimum 1 year of hospice nursing experience

Assignment Details

Length of Assignment: 13 weeks

Shift / Hours per Week: 8:30?am5:00?pm (Schedule can be flexible based on patients availability). One weekend per month.

Systems: Homecare, Homebase, HCHB

Start Date: ASAP
