Interim Healthcare is seeking a compassionate RN Case Manager for our Hospice Department. Territory is Shawnee, KS, KCKS and surrounding area(s).

Responsibilities RN Case Manager completes an initial/comprehensive assessment of the patient/family to determine hospice needs, including history of current and previous illnesses and a physical assessment with emphasis on management of pain and other symptoms.
Completes ongoing assessments of the patient’s physical status and the psychosocial status of the patient/family.
RN Case Manager initiates and develops a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician.
Makes necessary revisions to the IDG plan of care as the patient’s/family’s status and needs change.
RN Case Manager primarily responsible for the coordination of care among IDG members and other healthcare providers external to the IDG to ensure continuous assessment of patient/family needs and the implementation of the IDG plan of care to guide the delivery of services and achieve the goals of hospice care.
Minimum Requirements (Qualifications) Licensed to practice as an RN in the state MO and KS or have a compact license or be willing to obtain within 60 days of hire. License must be active and in good standing.
Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
Two (2) years of nursing experience preferred.
Able to provide proof of valid driver’s license.
Able to provide proof of valid auto liability insurance.
Must have a passion for Hospice.
