You'll play a crucial role in coordinating and delivering comprehensive care to patients in their homes, ensuring comfort and quality of life during their final stages.

Responsibilities Conduct detailed assessments of patients and their families.
Develop and manage individualized care plans.
Coordinate with interdisciplinary teams to provide holistic care.
Regularly review and update care plans based on patient needs.
Provide emotional support and education to families and caregivers.
Qualifications Certification: Licensed Registered Nurse with no restrictions.
Experience: Minimum 2 years in hospice care.
Skills: Exceptional communication and organizational skills.
Compassionate approach to patient care.
Proficiency in electronic medical records (EMR) systems.
