Responsibilities
Assess patient conditions and develop tailored care plans.
Maintain detailed medical records and communicate patient status to relevant parties.
Serve as the primary liaison among patients, their families, and healthcare providers.

Coordinate with healthcare professionals to ensure continuous care and efficient case management.
Requirements
License: Active RN license in Florida with no restrictions.
Experience: At least 1 yea r in a similar in-home healthcare role.
Skills: Proficient in case management software, exceptional organizational and communication skills.
