What You’ll Do As a Hospice RN Case Manager, you’ll bring comfort and dignity to patients and their families by:
Completing initial and ongoing assessments to determine hospice needs
Creating, updating, and implementing individualized care plans
Delivering skilled nursing care, medications, and symptom management
Supporting and educating patients and caregivers on comfort-focused care
Serving as case manager for your assigned caseload
Coordinating with physicians, families, and the interdisciplinary hospice team
Participating in on-call rotation and team meetings
Supervising and supporting hospice aides
What We’re Looking For Graduate of an accredited nursing program; current RN license in Missouri (or eligible)
1–2 years of recent acute care experience (home health/hospice preferred)
CPR certification
Valid driver’s license and insured, reliable vehicle
Excellent communication, assessment, and problem-solving skills
Compassionate approach to end-of-life care
