We are looking for RN Case Managers to provide per diem quality home visits, conduct PRN RN visits into individuals’ homes, and build one‑on‑one relationships with clients. Reliable transportation and the ability to travel to client homes are required.

Responsibilities

Provide per diem RN case management and quality home visits.

Perform per diem PRN RN visits into individuals’ homes and provide quality visits.

Build and develop one‑on‑one relationships with clients.

Implement the nursing care plan for each patient.

Provide nursing services, treatments, and diagnostic and preventive procedures as assigned.

Initiate preventive and rehabilitative nursing procedures as appropriate.

Observe signs and symptoms and report to the physician, including reactions and changes in the patient's condition.

Teach and counsel the patient and family/significant others regarding nursing care needs and related problems at home.

Maintain accurate and complete records of observations, treatments, and care of patient.

Participate in medical record audit as assigned.

Prepare clinical and progress notes.

Prepare equipment and materials for treatments.

Observe aseptic technique as required.

Assist the patient in learning appropriate self‑care techniques.

Qualifications

A valid license to practice nursing in the state of Ohio without restrictions.

A valid Driver's License and Auto Insurance.

Candidates must pass a BCI and FBI background check.

CPR / First Aid (or willingness to obtain upon hire).
