Job Description Our client in the Ventura area is looking to fill their RN Case Manager position!
Duties
Develop a care plan that is based on nursing diagnosis.
Initial assessment gets completed of patient and family to determine home care needs.
Re-evaluates patient nursing needs regularly.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
Initiates appropriate preventive and rehabilitative nursing procedures.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan.
Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
Perform other tasks as necessary.
Qualifications 2+ years of RN experience
Must have a valid driver's license
