Hospice RN Case Manager - Full‑time | Omaha, NE
Aegis Healthcare is expanding our Home Health & Hospice Nursing Team and is looking for a compassionate and experienced Home Health & Hospice RN Case Manager who is dedicated to improving patient care.
Employment Details
Compensation: $85,000 per year (average for RNs)
Employment Status: Full‑time
Schedule: Flexible. Monday–Friday, 8 AM – 5 PM

Your Role as a Hospice RN Case Manager
Comprehensive Patient Assessments – Evaluate physical, social, and psychological needs.
Collaborative Care Planning – Engage patients, families, and interdisciplinary teams.
Patient‑Centered Approach – Provide compassionate support to enhance comfort and quality of life.
Resource Coordination – Manage patient care resources and referrals efficiently.
Accurate Documentation – Maintain meticulous patient records to ensure seamless care.
Skills & Qualifications
Current RN license in Nebraska
Home Health & Hospice experience
Ability to assist with patient transfers using full range of movement
Strong verbal and written communication skills
Excellent time management and organizational abilities
Valid driver’s license and reliable transportation
Proof of auto insurance and registration
Current CPR certification (we can help obtain)
Negative TB test (we can provide testing)
HEP B record
Legal & Background Check
