Position Overview:
We are seeking a Field RN Case Manager for a client in Bethlehem. This role involves providing direct patient care and managing cases within the community to improve healthcare outcomes. Responsibilities:
Assess patient conditions and develop care plans.

Requirements:
Active RN license in Florida without restrictions.
Minimum of 1 year supporting in-home patients.
