Pay $80,000.00/yr - $90,000.00/yr
Description The RN (Registered Nurse) Case Manager provides case management and nursing services to Hudson Valley Hospice patients and their families and ensures that services are consistent with the philosophy, goals, objectives and policies of the Agency. Register Nurse (RN) Case Managers work in various settings: Nursing Homes, Home Care and In?Patient (throughout various hospitals).
Responsibilities
Perform initial and ongoing patient/family, physical, emotional, psycho?social, cultural and spiritual assessments to ensure and coordinate comprehensive, holistic services by the Interdisciplinary Hospice Team.
Provide palliative nursing care and symptom control in accordance with physician orders and the Individualized Dying Goals (IDG) plan of care.
Document and enter interventions of phone and home visits into EMR within 24 hours of the intervention; medication changes entered by end of shift.
Develop and prioritize interdisciplinary care plan goals of the patient/family, hospice team and attending physician prior to team meetings.
Update the care plan at a minimum every 14 days.
Provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur.
Teach, supervise and counsel patients/families/caregivers regarding physical care, observing return demonstration of procedures as appropriate.
Effectively communicate patient status to family members and/or caregivers.
Re?vise nursing diagnosis, goals and interventions, expected outcomes, and plan of care based on assessment of data and input of patients and family members prior to the IDG meeting.
Provide Coordinator or DPS and On Call staff reports on all changes in patient/family condition and Plan of Care.
Follow and uphold all state and federal laws and regulations outlined in the policy and procedure manuals.

Organizational and efficient time?management skills.
Bilingual is a plus.
Work Environment Work in various home environments, nursing homes or hospitals contracted with Hudson Valley Hospice.
Moderate/High level of possible exposure to infectious agents.
Moderate amount of travel by car.
Exposure to unsafe environments and direct contact with blood or body fluids.
Valid New York State drivers license with reliable transportation.
Education & Experience Graduate of an accredited nursing school.
Valid New York State nursing license.
Two years of nursing experience required.
One year of medical?surgery experience highly preferred.
Hours Monday?Friday (8:30am?4:30pm); on?call rotation (3?4 times per year)
