RN Case Manager - Home Care/Hospice (64 hours per period)
Base Pay Range
$38.19/hr - $57.29/hr
Summary
Implement and facilitate client plan of care. Communicates with other disciplines to provide direct oversight of client care.
Job Duties
Able to perform new client admission assessment accurately and complete within the 5 day guidelines.
Charting is clear and provides an accurate picture of the client.
Functions as a client advocate.
Makes referral to appropriate resources. Coordinates care between multiple/outside Care Teams.
Demonstrates effective client assessment and evaluation skills.
Functions as a positive role model in professional relationships and interactions.
Comments from MLHS employees, clients and families about positive attitude, professionalism etc.
Maintains 5 visits and documentation in an 8 hour day unless prior approved by manager/supervisor.
Practices infection control measures per standard.
Reports concerns of client safety to appropriate entity and provides necessary measures to promote safety.
Identifies needs and concern regarding safety and infection control.
Ensures cost effective client services.
Monitor and evaluate appropriate use of supplies.
Adheres to MLHS policies and practices including:

Participates in activities directed toward evaluating and improving service area functions.
Maintains a good record of attendance and punctuality.

Required Education and Experience
Graduate of an accredited school of nursing with current licensure as a registered nurse with the State of Minnesota.
Knowledge of Minnesota Nurse practice act and code of ethics.
Previous experience desirable.
Must successfully complete criminal background check as required by the State of Minnesota and Drug/alcohol screening per MLHS policy.
Must have a valid driver's license.
Must have CPR/BLS certification.

Position Type / Expected Hours of Work
This position is scheduled Monday through Friday eight hour day shifts with rotating call for weekends and holidays.
Supervisory Responsibility
Supervisory responsibilities will include oversight/management of licensed and non-licensed personnel in Home Care and Hospice.
