Position Summary
The Case Manager accomplishes patients’ care by assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches, coordinating and providing care that is safe, timely, effective, efficient, and patient‑centered.

Required Qualifications

All external candidates must have a Bachelor of Science in Nursing.

Current California RN license.

American Heart Association BLS Certification.

Three to five years of experience in an acute care hospital setting (medical/surgical preferred).

Previous one to two years of experience as a Case Manager.

Must be proficient in typing, writing and communicating professionally.

Must be proficient in computer skills including Microsoft Office.

Must possess the skill, knowledge and ability essential to the successful performance of assigned duties.

Must demonstrate customer service skills appropriate to the job.

Excellent written and verbal communication skills in English.

Ability to maintain a work pace appropriate to the workload.

Ability to establish and maintain effective working relationships across the Health System.

Preferred Qualifications

Broad knowledge of Medicare, Medi‑Cal and insurance guidelines.

Critical care experience (For Resource Case Manager).

Knowledge of University and medical center organizations, policies, procedures and forms.
