Schedule: Monday-Friday 8:30AM-5:00PM with On-call requirements
Service Area: Vermillion, Amherst, South Amherst, Sheffield Lake and surrounding areas
HOW YOU'LL MAKE A DIFFERENCE: As a Registered Nurse Case Manager (RNCM), you will make a difference in the lives of our patients by providing home-based nursing care, coordinating care with an interdisciplinary team, and supporting patients' recovery journeys. You will work autonomously, assess patient conditions, develop care plans, and educate patients and families.

HOW YOU'LL WORK: Lead patient assessments, manage care plans, educate families, and ensure patient safety while adhering to state scope of practice. Manage your time efficiently and collaborate with team members.
MAJOR AREAS OF RESPONSIBILITY:
Plan of Care: Monitor patient conditions, perform assessments, and develop individualized care plans.
Minimize Patient Risk: Administer medications and prioritize high-risk visits.
Supervision: Oversee care provided by nurse aides and LPNs.
Continuity of Care: Coordinate admissions, transfers, and discharges.
Family Educator/Advocate: Educate patients and families using teach-back methods.
Policies: Complete documentation and collaborate with the team.
Rules and Regulations: Follow policies and communicate schedule changes.
Operations: Attend training and meetings.
HARD & SOFT SKILLS: Compassionate communication, patience, attention to detail, observant, and able to follow directions.
REQUIREMENTS: Current RN license, minimum one-year acute care experience (home care preferred), valid driver’s license and auto insurance, capable of physical demands.
