Position Highlights

Shift Schedule: Full time, Days, rotating between three 12-hour shifts one week and four 12-hour shifts the next, every other weekend required

Department: Care Coordination – Coverage Area: Emergency Department

Location: Winston Salem Campus

What You'll Need

Graduation from an accredited School of Nursing; BSN required. MSN preferred.

Registered Nurse licensure from the North Carolina Board of Nursing

Minimum 2 years of relevant clinical experience; Case Management experience preferred

Basic Cardiac Life Support (BCLS) required

Certification in Case Management highly recommended within 2 years of employment

How You'll Impact Patient Care

You will lead a dedicated team and collaborate with a diverse group of healthcare professionals to provide age-appropriate and developmentally sensitive care that meets high nursing standards.

Job Responsibilities

Identify patients who would benefit from Case Management interventions based on discharge needs

Assess data by interviewing patient/family and evaluating needs

Develop and implement a Plan of Care with nursing to improve outcomes; coordinate discharge planning with the multidisciplinary team

Evaluate and monitor discharge planning effectiveness and adjust the Plan of Care as needed

Document Case Management actions in the Electronic Medical Record and communicate with providers and payors

Participate in multi-disciplinary rounds and collaborate with physicians to facilitate patient progression

Identify system issues and suggest interventions to improve efficiency and reduce variances

Monitor medical necessity and notify Utilization Review as needed

Apply quality improvement principles to reduce variations in practice

Provide age/developmentally appropriate communication following Age-Specific Care Guidelines

Understand payer and reimbursement practices and guide patients through post-discharge options
