Join MedStar Health as a RN Case Manager (Discharge Planner) and support patients through the continuum of care in an acute hospital setting.
Position Summary
This role serves as a member of the Case Management Team, applying RN clinical expertise and medical appropriateness to care coordination and discharge planning. You will facilitate the delivery of high quality, cost‑effective, patient‑centered care from pre‑admission through post‑discharge, ensuring individualized patient outcomes and optimal resource utilization.
Primary Duties And Responsibilities
Contribute to the achievement of department goals and adhere to policies, quality standards, safety standards, and accreditation regulations.
Collaborate with the multidisciplinary health‑care team to develop and coordinate the plan of care.
Communicate with patients, families, healthcare team, external case managers, community resources, and facility to address issues and patient/family goals.
Develop a plan of care that addresses continuum needs, interventions for identified problems, long‑ and short‑term goals with time frames, specific services, and family involvement, including life‑planning contingencies such as power of attorney and advance directives.
Evaluate and document patient responses to the plan of care, recommend modifications, and adhere to documentation and confidentiality policies.
Maintain knowledge of regulatory agencies’ admission criteria and Medicare/Medicaid reimbursement methods for various care settings.
Manage a caseload of patients and identify essential resources to implement the plan of care.
Advance professional growth in managed care, financial trends, clinical practice, and research.
Direct patient care decisions according to multidisciplinary plans and managed‑care contracts, managing barriers that impact outcomes.
Participate in performance and service‑improvement teams, assist program evaluation through surveys, LOS data analysis, charge/discharge data comparison, and best‑practice benchmarking.
Perform comprehensive assessments with interdisciplinary teams to identify patient‑specific problems and needs related to diagnosis, treatment, psychosocial and financial concerns.
Minimal Qualifications
Education
Associate’s degree in Nursing (ADN) required.
Bachelor’s degree in Nursing (BSN) preferred.
Experience
2‑3 years of clinical experience required.
1‑2 years of case‑management experience preferred.
Licenses and Certifications
Registered Nurse (RN) state licensure or compact licensure in the District of Columbia or Maryland (required).
Certified Case Manager (CCM) preferred.
Knowledge, Skills and Abilities
Ability to use computers to enter and retrieve data.
Working knowledge of Microsoft Word, Excel and PowerPoint.
Effective verbal and written communication skills.
Capability to run and analyze departmental productivity reports.
Excellent interpersonal skills.
Salary Range
USD $87,318.00 – $157,289.00 per year.
