About The Position
Goodwin Home Health is currently seeking a full‑time Clinical Nurse Manager responsible for leading the nursing team to include RNs, LPNs, HHAs. The Clinical Nurse Manager supervises the RNs and supports the case management of patients receiving skilled nursing care. This position supports Home Health Leadership administratively by reviewing clinical information to assure all OASIS documentation is reviewed, accurate, and appropriate per CMS/ACHC guidelines and for billing purposes. The Clinical Nurse Manager also assists with nurse scheduling, obtains necessary orders and documentation from MD offices, and collaborates with the Director of Clinical Services to ensure the interdisciplinary team is working toward patient‑centered care and goals to support the mission of Home Health at Goodwin Living. Goodwin Home Health provides skilled care throughout Northern Virginia, focusing on the counties of Arlington, Alexandria, Fairfax, Prince William, and Loudoun.
Job Duties
Perform / complete OASIS documentation
Perform detailed assessment resulting in the development of a patient‑specific Plan of Care (patient clinical record documentation done per agency policy)
Provide direct nursing care in a Home Health setting under the guidelines of a physician’s order
Communicate with other team members regarding the Plan of Care and work closely with team members for optimal outcomes
Effectively communicate with referring physicians and provide accurate treatment notes and other necessary documentation in a timely manner, per policy
Prepare for patient discharge planning as soon as possible and inform and prepare the patient, team members, and physician
Qualifications
Registered Nurse with an active, current, and unrestricted license (required)
Minimum of two (2) years of home health experience
Minimum of two (2) years in a clinical setting
Strong knowledge of Medicare/Medicaid guidelines, PDGM coding and process, OASIS accuracy/coding, and CMS/ACHC guidelines and requirements
Proficient analytical, negotiation, communication, problem‑solving and decision‑making skills (required)
Proficient in Microsoft Office and Excel, willing to adapt to various EMR systems
