Overview RN CASE MANAGER - UNM Hospital
Position Summary Coordinate all systems/services required for an organized, multidisciplinary, patient-centered care team approach, and assure quality, cost-effective care for the identified patient population. Manage the course of treatment of patients, coordinating care with physicians, nurses and other staff to ensure quality patient outcomes within established time frames and with efficient utilization of resources. Conduct initial and ongoing assessments, initiate disease management protocols, determine and manage outcomes, ensure continuity of care through discharge planning, utilization of resources and analysis of variances. Function as a contact person for patient, family, health care team members, community resources and employees as necessary. Adhere to Hospitals and departmental policies and procedures. Patient care assignments may include neonate, pediatric, adolescent, adult and geriatric age groups.
Responsibilities DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops
VARIANCE - Review variance from standardized protocols of care with health care team members and implement resolution strategies
REPORTS & RECORDS - Maintain computer-based tracking system and compile required reports and records
QUALITY - Participate in continuous quality improvement activities by evaluating patient care systems and documentation
COMMITTEES - Attend meetings and represent department or hospitals within committees or the community, as assigned
EDUCATION - Provide instruction to patient and family based on learning needs; assess knowledge and expectations
INFORMATION - Help develop and disseminate information to health care team
DATA - Perform assessment, data collection, and analysis in collaboration with patient, family and team
ASSESSMENT - Assess patient’s clinical, psychosocial status and current treatment plans
COMMUNICATION - Use interpersonal communication to achieve outcomes and satisfaction
IDENTIFICATION - Identify appropriate patients requiring case management interventions using established procedures
GOALS - Assist patient and family to set patient-centered goals with caregivers and health care team
PLAN OF CARE - Develop comprehensive multidisciplinary plan of care
VARIANCES - Intervene when variances occur in treatment plans
RESOURCES - Coordinate and evaluate resources for quality and cost-effective outcomes
DISCHARGE PLANNING - Conduct timely discharge planning in collaboration with care team
INTERVENTIONS - Monitor and evaluate responses to interventions with follow-up
TREATMENT CONFERENCE - Participate in conferences to evaluate interdisciplinary dynamics and management
CONTINUITY OF CARE - Ensure continuity of care across settings; promote effective communication
MEETINGS - Participate in team meetings as indicated
CARE PLAN - Incorporate recommendations of interdisciplinary team in the care plan
COLLABORATION - Develop relationships with departments and community health care agencies to support quality care
DOCUMENTATION - Provide timely documentation of initial assessment and progress
ORIENTATION - Participate in orientation and continuing education of staff
NEEDS - Assess needs related to diagnosis and resources; coordinate options, including discharge planning
ORDERS AND REFERRALS - Obtain necessary orders to initiate home health referrals and equipment; coordinate referrals
PATIENT CENTERED MED - Adhere to Patient Centered Medical Home/Patient Centered Specialty Practice expectations
PATIENT SAFETY - Follow safety policies and proactively seek improvements
MEDICATION - Administer medication when applicable; document and communicate findings
PATIENT CARE - Coordinate nursing care and related activities; may facilitate group therapy/education sessions
Qualifications Education :
Essential: Program Graduate
Nonessential: Bachelor’s Degree
Education specialization :
Nationally Accredited Nursing Graduate
Experience :
Essential: 1 year directly related experience
Nonessential: Bilingual (English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo)
Credentials :
Essential: RN in NM or as allowed by reciprocal agreement by NM
CPR for Healthcare/Basic Life Support Provider or Professional Rescuer w/in 30 days
Physical/Working Conditions :
Light Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently; may involve standing/walking
Working conditions: Minor hazard; TB testing required upon hire and as needed

Compensation :
Minimum Offer: $38.56/hr.
Maximum Offer: $49.48/hr.
Compensation varies based on factors including experience and education.
